SMS Terms & Privacy

1. Scope

This policy applies to appointment reminders and closely related administrative communications sent by or on behalf of Core Chiropractic through SMS/MMS text messages, telephone calls or voicemail, and email. It supplements any other privacy notices that apply to our website, healthcare operations, or patient records.

2. Information We Collect

For the messaging program, we may collect and maintain:

• Your name
• Your mobile telephone number, telephone number, and email address
• Your communication preferences and consent status
• Appointment date and time and limited scheduling information
• Messages you send to us and our responses
• Message-delivery, opt-in, and opt-out records
• Limited technical and service information supplied by our communications provider

We design appointment reminders to avoid diagnoses, treatment details, test results, or other clinical information. However, any communication with a healthcare provider may be sensitive, and the name of our practice in a message may reveal that you have a relationship with us.

3. How We Use Information

We use messaging information to:

• Send appointment confirmations, reminders, scheduling updates, and office-closure notices
• Respond to scheduling or administrative questions
• Record and honor communication preferences and opt-out requests
• Operate, secure, troubleshoot, and improve the messaging program
• Comply with legal, regulatory, carrier, and recordkeeping requirements

We do not use consent to appointment reminders as consent for advertising or promotional text messages.

4. How We Share Information

We may share information with vendors that help us operate the messaging program, including Nextiva and telecommunications carriers, solely as needed to provide, deliver, secure, or support communications. We may also disclose information when required by law, to protect rights or safety, or in connection with a permitted business reorganization.

5. Data Security and Retention

We use reasonable administrative, technical, and physical safeguards appropriate to the sensitivity of the information. No email, telephone, or text-message system is completely secure. Messages may be seen by anyone with access to your device, account, voicemail, or email, and ordinary SMS/MMS may not be encrypted end to end.

We retain consent, opt-out, delivery, and communication records for as long as reasonably needed for operations, legal compliance, dispute resolution, and enforcement of agreements, and then dispose of them in accordance with applicable policies and law.

6. Your Choices

Participation in our SMS/MMS messaging program is voluntary and is not a condition of receiving treatment or services.

By providing your mobile telephone number and expressly opting in to receive text messages from Core Chiropractic Center and Rehab, you consent to receive SMS/MMS messages regarding appointment confirmations, appointment reminders, scheduling and rescheduling updates, office notices, and other closely related administrative communications.

Message frequency varies based on your appointment and scheduling activity. Message and data rates may apply.

After opting out, you may still receive communications that are legally required or that you separately request through another communication channel. You may ask us to use a reasonable alternative method to contact you.

7. Children and Minors

For a patient who is a minor or otherwise cannot provide consent, a parent, legal guardian, or other legally authorized representative must provide the contact information and consent. The representative confirms that they are authorized to receive appointment communications concerning the patient and must notify us if that authority or the telephone number or email address changes.

8. Changes to This Policy

We may update this policy from time to time. The updated version will be posted with a revised effective date. Material changes will be communicated when required by law.
